Donors with an employer authorization form
Donors are required to bring photo ID/Drivers License and a unexpired authorization form.
You must request a new authorization form from your employer. Expired authorization forms will not be accepted.
Depending on the type of drug screen requested by an employer, a donor can expect to provide a urine sample between 30mL to 45mL. Some drug screen require direct observations.
We encourage donors to limit excessive personal items and large bags, purses and backpacks as locker space is limited. Donors will be asked to return excessive possessions to their vehicle.
Donors with an employer authorization form should expect results within 24-48 hours. Results are provided to the donor directly by the employer.
