Donors are required to bring photo ID/Drivers License and a unexpired authorization form.
You must request a new authorization form from your employer. Expired authorization forms will not be accepted.
Depending on the type of drug screen requested by an employer, a donor can expect to provide a urine sample between 30mL to 45mL. Please drink plenty of water prior to your visit.
